A Utah wiki that has teacher created math tasks. The tasks are organized by standard and are downloadable.
National Library of Virtual Manipulatives 
The National Library of Virtual Manipulatives (NLVM) is an NSF supported project that began in 1999 to develop a library of uniquely interactive, web-based virtual manipulatives or concept tutorials, mostly in the form of Java applets, for mathematics instruction (K-12 emphasis). Pick your grade level and math area to select digital manipulatives. 
K-5 Math Teaching Resources 
A site that provides an extensive collection of free resources, math games, and hands-on math activities aligned with the Common Core State Standards for Mathematics. There are also Math Journal Tasks (K-5), Math Vocabulary Resources (Grds. 2-5) and Math projects (Grds. 3-5).
